What is an Advance Directive?

An Advance Directive (AD) is a document that lets you plan your health care ahead of time. It’s useful if you ever become too sick or hurt to make decisions about your own care.

What Can an Advance Directive Do?

With an advance directive, you can:

  • Choose someone you trust to make health care decisions for you (called an “agent”).
  • Share your preferences about the kind of care you want or don’t want.
  • Do both: name an agent and give specific instructions about your care.
