Discovery Place: Transitional Housing for Recovery
Discovery Place provides a supportive home for people dealing with mental health or substance use issues, helping them continue their recovery in a structured community setting. The house is staffed 24 hours a day and can accommodate up to 8 residents of any gender.
At Discovery Place, we focus on helping residents work towards their personal goals and become more independent. The services we offer include:
- Daily Living Assistance: Help with everyday tasks.
- Care Coordination: Organizing and managing care.
- Medication Support: Assistance with medication needs.
- Social Skills Enhancement: Improving social skills.
- Transportation Navigation: Help with getting around.
Subsidy Program
The housing subsidy program is financial help for people with severe mental illness who are getting services from MPNN BH. The payment goes directly to the landlord to cover part of the housing cost. People who get this help must still contribute to their housing costs. We need to verify income to determine the subsidy amount. This program helps individuals live independently in their community.
HUD Housing Program
MPNN BH is excited to team up with the Department of Housing and Urban Development (HUD) to provide housing for people with disabilities. We offer different options for safe and supportive housing that help individuals live independently and comfortably in their community.
