Assertive Community Treatment (ACT) is a program offered by a team of experts in mental health and substance use. The team includes:
Psychiatrists
Nurses
Team Leaders
Co-Occurring Specialists
Vocational Specialists
Peer Recovery Specialists
Generalists
ACT services are provided at home or in the community to help people fully participate in their communities. This is an intensive service available 24/7, including on holidays and weekends. Currently, ACT is only available in the Middle Peninsula.
ACT Services Include:
Organizing and supervising medication to ensure it's taken correctly
Help with daily living tasks
Work-related services
Supportive counseling
Teaching social skills
Peer counseling and support
Educating families
Who Uses ACT?
People who usually need ACT have:
Schizophrenia, bipolar disorder, or major depression with psychotic features
Significant difficulties with daily living, work, home life, or staying safe
One or more of these issues:
Substance abuse
Frequent or long-term hospital stays
Frequent arrests, jail time, or homelessness
They have not done well with other community-based treatment programs
